Request Patient ID Number

To request a system email with your patient ID number, please use the retrieval option on the OBERD supported login page.

STEP 1:

Once you’re at the patient login page, click the Send Me My Patient ID link.

A system generated email will be sent with the information to your email address on file. If you have not received the email within 30 minutes please check your spam or junk folder. You can also request your patient ID from the staff at your Provider's office. Our technical support staff is unable to give out any patient information through email.

Security Questions

To login to the OBERD supported portal all you need to use is your first name, last name, DOB, last four of your SSN or patient ID, and then security questions. There should not be a password required to log in.

If the system is prompting you with security questions you don't recognize, or not recognizing the answers you submit, you have the ability to reset the security questions on your end.

STEP 1:

Click the Reset Security link on the Security Question Screen.

A reset email will be sent to the address associated with your account. The screen should then take you back to the Security question page, but you must reset them using the email that you are sent through the system, first. If you have not received the email within 30 minutes please check your spam or junk folder. Please remember that security answers are case sensitive.

Medical Issue

Please contact your provider’s office directly for any medical issues, appointment requests or inquiries, or to otherwise communicate with them. Our technical support staff does not have access to your personal or medical history, billing, or insurance information, and we do not have direct contact with your provider’s office.

Login Info Not Accepted

Contact your Provider's office directly to request that they update your current account information. To ensure security and consistency all account updates must be made through the staff at your Provider's office. Our technical support staff has limited access to what information is available to us and we are unable to change or update it accordingly.

Change Account Info

Contact our Provider's office directly to request that they update your current account information. New Reminder Email Sent

Contact your Provider's office directly to request that they send a new reminder email. Once completed you will receive a new reminder email with an updated link for your requested online forms. This should allow you to login and complete the questionnaires normally. If you are still experiencing an issue with accessing form information, then please let them know. If you have not received the email, or do not receive the email within 30 minutes of the representative sending it, then I would recommend double checking your spam or junk email folder.

Accessing Physician Notes

The best way to access your physician notes will be to locate ‘MENU’ in the upper right of the screen next to the patient’s name. You will then select ‘Medical Records’.

If the physician has left notes for the patient’s visit, you will find them here, otherwise the physician has not shared any notes. For any other inquiries, I encourage you to reach out to your provider’s office directly for any medical issues, appointment requests or inquiries, or to otherwise communicate with them. Our technical support staff does not have access to your personal or medical history, billing, or insurance information, and we do not have direct contact with your provider’s office.

Form Specific Questions

For form specific information I would recommend contacting your Provider's office directly. They actually control assigned forms and they are better equipped to assist you with answering questions regarding the content of your forms and to provide assistance with why they might have been assigned to your account. They can also remove any incorrectly assigned or redundant forms. To protect patient privacy and security, our technical support staff is unable to change or update any form information.

Unsubscribe

The staff at your institution actually controls assigned form and reminder information, so they'd be better equipped to assist you with your request if you would prefer to be removed from receiving reminder emails altogether, but you also have the option to unsubscribe from some or all of the emails by completing the following steps:

STEP 1:

At the very bottom of the email there will be the Unsubscribe link.

From the next page you will need to unselect the appropriate boxes and verify date of birth to no longer receive those emails.

Subscription settings

This option will not prevent further communication with your institution. It will simply prevent you from receiving excessive reminder emails.

You can also log into the OBERD supported portal to turn off reminder emails within the Reminder Settings option on the Home Screen menu:

Click on Menu button.

Under that option you should find some setting tabs that you can adjust to turn reminders on and off.

Click Save Preferences when completed to update your account.
